ISSUE OF PASSPORTS IN LIEU OF PASSPORTS/ TRAVEL DOCUMENTS REPORTED LOST / DAMAGED

1. Applications for new passports in lieu of a lost/damaged passport, should be accompanied by the following documents.
   
   a. Form "K" (in duplicate)
   b. Photo/Signature Card (IM35 B) with 3 colour photographs of size 35mm x 45mm
   c. Declaration form (Form D). Please write in Block letters.
   d. Application for addition/deletion of children's names where necessary.
   e. Birth certificate (Original required)
   f. National Identity Card (Original required)
   g. Marriage certificate where applicable (Original required)
   h. Written confirmation from the relevant area police to the effect that the loss of passport has been reported (Police report)
   
   
2. The fee for obtaining a new passport in lieu of a lost passport is US$ 353.00. This includes administrative charges of US$ 200.00 and levy of US$ 153.00 in addition to the postage charges.
   
   
3. If the return of the passport is required by Mail, applicants in the USA may choose one of the following options and make payment together with the application fees.
   
   Federal Express - $ 20.00
   US Postal Express mail - $ 18.00
   Registered Mail - $ 12.00
   
   
4. Fees should be paid by Cash (only if application is hand delivered). The Money Orders or Certified Cheques made in favour of "Embassy of Sri Lanka". Please note that personal cheques will not be accepted.

Notes

i. This Embassy is not responsible for any document lost in the post.
ii. Submission of forged documents and making false declarations relating to citizenship is a punishable offence. Fees will not be refunded in these circumstances.
iii. Incomplete applications will be rejected.
