ISSUANCE OF EMERGENCY TRAVEL DOCUMENTS - (NON-MACHINE READABLE PASSPORTS)

  1. A temporary travel document (Non-Machine Readable Passport) valid for six months may be issued to enable a Sri Lankan national to return to Sri Lanka only, due to an emergency, in the following circumstances.

    • The original passport is lost / damaged. (A Police Report and forms D2 and D3 should be submitted in addition to the requirements under 3.)
    • Passport has expired and cannot be extended.
    • Inadequate time to obtain a new passport.

  2. If the applicant is already in possession of a passport, it will be cancelled prior to the issuance of the Non-Machine Readable Passport (NMRP).

  3. Applications for NMRPs should be accompanied by the following documents.

    a. Form "K" (in duplicate)
    b. Photo/Signature Card (IM35 B) with 3 colour photographs of size 35mm x 45mm
    c. Declaration form (Form D). Please write BLOCK letters.
    d. Application for addition/deletion of children's names where necessary.
    e. Birth certificate (original required)
    f. National Identity Card (original required)
    g. Marriage certificate where applicable (original required)
    h. Travel itinerary

  4. A fee of $ 50.00 will be charged for the issuance of a NMRP, when the applicant is in possession of a valid passport.

  5. If the applicant has lost his passport, a fee of US $ 150.00 will be charged for the issuance of a NMRP. This includes a 'lost passport' fee.
  6. If the return of the passport is required by Mail, applicants in the USA may choose one of the following options and make payment together with the application fees.

    Federal Express - $ 20.00
    US Postal Express mail - $ 18.00
    International mailing fee - $ 35.00

  7. Fees should be paid by Cash (only if application is hand delivered). The Money Orders or Certified Cheques made in favour of
    "Embassy of Sri Lanka". Please note that personal cheques will not be accepted.
