APPLICATION FOR AMENDING A SRI LANKAN TRAVEL
DOCUMENT
(A) Alterations
- Applications for alteration of a passport should be made
on Form 'O' (IM 37) which may be obtained from this Embassy
by sending a stamped self addressed envelope.
- Form 'O' should also be used when a person requires an endorsement
in the passport regarding the following:
a. Change of name consequent to marriage (original or notarized
copy of marriage certificate is required. Marriage certificates
issued in Sri Lanka should be certified by the Consular Division
of the Ministry of Foreign Affairs, Colombo.)
b. Change of name (provided the name in the birth certificate
has been altered in accordance with the birth registration regulations
of Sri Lanka and the relevant birth certificate which has been
notarized should be submitted).
c. Change of employment (letter from employer or similar proof
required).
d. Include or Change National Identity Card (NIC) number. (The
original National Identity Card should be provided)
e. If a passport has been issued with a restricted endorsement
and such endorsement has to be amended or deleted, the passport
should be submitted along with relevant supporting documents.
- If the applicant wishes to change his / her signature or photograph
due to a change in appearance, a new passport should be obtained.
- Please do not affix any stamps. The applicant should place
his signature in the cage at the bottom of page 1
- The Consular fees for alteration of a passport is US $ 10.00.

- If the return of the passport is desired by Mail, applicants
in the USA may choose one of the following options and make
payment with the relevant application fees.
Federal Express - $ 20.00
US Postal Express mail - $ 18.00
Registered Mail - $ 12.00
- Fees should be paid by Money Orders or Certified Cheques made
in favour of "Embassy of Sri Lanka". Please
note that personal cheques will not be accepted.
- When the application is duly completed, it should be submitted
to the Embassy with the following:
a. Passport to be altered
b. Supporting documents for the alterations
c. Consular fee and postage
Notes
i. This Embassy is not responsible for any document lost in the
post.
ii. Submission of forged documents and making false declarations
relating to citizenship is a punishable offence. Fees will not
be refunded in these circumstances.
iii. Incomplete applications will be rejected.
(B) Renewal
- Applications for alteration of a passport should be made on
Form 'O' (IM 37) which may be obtained from this Embassy
by sending a stamped self addressed envelope.
- The validity of a passport can be renewed only up to a period
of 10 years from the original date of issue. Please use Form
'O' for this purpose. The validity of passports which are
over ten years from the date of issue cannot be further extended.
- In the case of valid passports in which all pages are exhausted,
a new passport may be obtained for which Form 'K' should
be used. Please note that extra pages cannot be added
to such passports.
- The fees for renewal of passports are as follows: (Amended
with effect from 04 April 2008)
One Year - $ 6.00
Two Years - $ 12.00
Three Years - $ 18.00
Four Years - $ 24.00
Five Years - $ 30.00
- If the return of the passport is desired by Mail, applicants
in the USA may choose one of the following options and make
payment with the relevant application fees.

Registered Mail - $ 12.00
- Fees may be paid by cash (if application is hand delivered).
Money Orders or Certified Cheques should be made in favour of
