APPLICATION FOR ADDITION / DELETION OF CHILDREN
  1. Children who are below 16 years of age and whose births have been registered with the Embassy may be included in the passport of either parent.

  2. Children whose names appear in the passport of either parent may be included in the passport of the other parent instead, if a request is made to this effect.

  3. The following documents should be produced along with photocopies. Originals will be returned after perusal.

    a. Travel document of the applicant (i.e. Passport/Emergency Certificate/Identity Certificate)

    b. Photocopies of the page for inclusion of children's names, and the page containing particulars of the spouse's passport. If the spouse does not posses a travel document an affidavit to that effect.

    c. Proof of citizenship of father.

    d. Birth certificate/s of child/children. (original required)

    e. Parents' marriage certificate (original required)

    f. If divorced, divorce certificate and documents to prove the legal custody/guardianship of child/children. (original required)

  4. When adding the names of children to the parent's passport, the written consent of the spouse is required. If the spouse is abroad, a written declaration should be forwarded.

  5. 03 colour photographs of each child (size 2.5 cm x 3 cm) with the name written on the reverse of each photograph is required. One of the photographs should be attested by a Notary Public or an Attorney at Law.

  6. No additional fee will be levied if the names of the children are included at the time of applying for a new passport. For later additions or deletions US $ 10.00 will be charged. (Amended with effect from 04 April 2008)

  7. If the return of the passport is required by Mail, applicants in the USA may choose one of the following options and make payment with the application fees.

    Federal Express - $ 20.00
    US Postal Express mail - $ 18.00
    Registered Mail - $ 12.00

  8. Fees should be paid by Cash (only if application is hand delivered). The Money Orders or Certified Cheques made in favour of "Embassy of Sri Lanka". Please note that personal cheques will not be accepted.

Notes

i. This Embassy is not responsible for any document lost in the post.
ii. Submission of forged documents and making false declarations relating to citizenship is a punishable offence. Fees will not be refunded in these circumstances.
iii. Incomplete applications will be rejected.
